2025-26 L & I Parent/School Authorization Form

This form needs to be filled out every new school year. Previous forms are not valid for the current school year. Please fill out completely front and back, with all necessary signatures from student, parent, and employer, and bring to school to have a copy made. The original needs to go to the employer. These forms can also be found in Wa-Hi's front office, Room B-117 (Ma's classroom), and Room S-101 (Stout's classroom).
